What is your new patient procedure
You will fill out the necessary patient information and demographics, which can be sent to you by text or e-mail at the time of your scheduling an appointment.
What about needed lab work or X-rays?
We are connected with a third-party lab and X-ray facility to meet your needs at prices that you can afford.
What are your hours?
Monday through Friday, 8 am to 8 pm CST, Saturday 10 am to 6 pm CST. With after-hours available (for an additional fee).
Where are you located?
We are a primary telehealth service, however, you may be seen in our office located at Lavish Med Spa 9660 Bartlett Circle Suite 700, Fort Worth, Tx 76108, or we can arrange home visits (for an additional fee).
Do you accept insurance?
We are currently working on being able to accept Medicare and Blue Cross Blue Shield, However, At this time, you can pay our affordable price and submit your bill and supporting documentation to Medicare or your insurance company for Reimbursement.
